When precision reaches the patient: scaling personalised, multidisciplinary supportive cancer care, 2020–2026 (146505)

Aims: Precision medicine promises the right treatment for the right person, yet a targeted therapy that never reaches the patient changes nothing. Exercise medicine and multidisciplinary supportive care are precision therapies in their own right. Guideline-recommended to be individualised and matched to each person's cancer, treatment and needs, yet remain inconsistently delivered. Embedding them in routine care is as urgent as translating any emerging molecular therapy. We report a real-world model delivering this precision-matched, integrated care in line with the COSA position statement on exercise in cancer care.

Methods: Descriptive analysis of service data from the Lift Model of Care, a clinician-led supportive oncology service (Adelaide; reliable data from 2020). Individually prescribed exercise medicine is delivered alongside physiotherapy, exercise physiology, lymphoedema screening and treatment, dietetics and psychology. For 2020 to 30 June 2026 we report unique patients, episodes of care, and patients accessing two, three or four-plus services annually.

Results: Between 2020 and June 2026 the service delivered 77,712 episodes of care, with exercise medicine the largest stream. Across 2020 to 2025, annual episodes rose from 6,447 to 17,172, reach from 608 to 1,309 patients, and multidisciplinary care (two or more services) from 44.6% to 53.0%  the majority of patients since 2024, while those combining three or more services grew from 24% to 32%. This continued through the first half of 2026: 10,566 episodes, 1,093 patients, 50.5% accessing multiple services and 29% three or more, in six months.

Conclusions: This is precision realised at the point of care: guideline-recommended, individually tailored supportive care, delivered and scaled in routine practice. The model shows how services can move exercise and multidisciplinary care from recommendation to reliable delivery for every patient.
