Aims: To describe the implementation and integration of a patient reported measures (PRMs) screening program with longitudinal symptom monitoring across two cancer centres, translating implementation strategies and lessons learned from the PROMPT-Care Pilot Study into routine clinical practice.
Methods:
Participation in the PROMPT-Care Pilot study, a non-randomised controlled trial of ePROM collection across 4 cancer centres, was a key driver in the design and basis for the statewide Cancer Institute NSW (CINSW) Patient Reported Measures Program. Key implementation strategies included strong clinical leadership, multidisciplinary collaboration, workflow redesign, integration into routine care, and establishment of a cross-site governance group. Ongoing clinician engagement, education, and iterative workflow refinement supported sustainable adoption across both sites.
Results:
The PRMs program is embedded within routine radiation oncology practice across both cancer centres. Since implementation in October 2021, 13,182 PRM surveys have been completed by 4591 patients within Illawarra Shoalhaven Local Health District representing one of the highest levels of PRMs participation in NSW. Routine longitudinal screening improved visibility of symptom burden and psychosocial distress, supporting earlier identification of unmet needs and timely multidisciplinary intervention and personalised supportive care. Sustainability has been supported through ongoing staff engagement, education, governance oversight and iterative workflow refinement. Key implementation challenges included workforce capability, patient digital literacy and resource constraints, which were addressed through targeted education and process improvements.
Conclusion:
Successful implementation of an ePROMs program across two regional cancer centres demonstrates that routine longitudinal symptom monitoring can be sustainably integrated into clinical practice. The system has enhanced visibility of patient needs and supported timely multidisciplinary intervention, providing a scalable model for patient-centred supportive care delivery. Future work will focus on expanding clinician engagement across radiation and medical oncology and further integrating PRM data into follow-up care pathways.
