Personalised Dosing of Cancer Medicines in Clinical Practice (146414)

Precision oncology has transformed cancer treatment through molecular profiling and biomarker-driven therapy; however, most systemic anticancer medicines continue to be administered using fixed or body surface area (BSA)-based dosing. This conventional approach fails to account for the substantial inter-patient variability in drug exposure resulting from differences in variables such as  pharmacokinetics, pharmacogenomics, and organ function. Consequently, many patients experience either avoidable toxicity or inadequate drug exposure, potentially compromising treatment outcomes.

This presentation will explore the clinical applications of different individualised dosing strategies across cytotoxic chemotherapy, targeted therapies, and immunotherapy. Key strategies including therapeutic drug monitoring, pharmacogenomic-guided prescribing, toxicity-guided dose adaptation, and biomarker-driven dosing will be discussed. Practical examples from clincial practice will demonstrate how dose individualisation can improve treatment efficacy while reducing adverse events and unnecessary healthcare utilisation.
