From the Ward to the Community: An Active Approach to Childhood Cancer (The MERRIER Study) (146190)

Aims: Despite the benefits of physical activity, most childhood cancer survivors are not sufficiently active. The MERRIER Study aims to examine the acceptability and feasibility of a fully online delivered exercise program for childhood cancer survivors.

Methods: This type-1 hybrid effectiveness-implementation trial recruited participants aged 5-18 years following completion of cancer treatment through community organisations (Camp Quality and Koala Kids) and Westmead Children’s Hospital (target n=60). Participants were randomised to a 12 week online exercise intervention or usual care with a delayed intervention offered at 6 months. The intervention comprised 5 video calls with an Accredited Exercise Physiologist and an individualised home exercise program. Exercise prescription was tailored to participants’ goals and abilities, and engagement was enhanced through online resources and games. Physical activity (accelerometry) was measured at baseline and 3 months, while functional outcomes (virtual physical assessments) and patient reported outcomes were assessed at baseline, 3 and 6 months. Implementation outcomes were evaluated using the RE-AIM framework.

Results: A total of 39 participants were recruited; 37 were randomised (21 intervention, 16 control). Median age was 8 years (range 5-16), 62% were male, and acute lymphoblastic leukaemia was the most common diagnosis (46%). No adverse events were reported. Attendance was high, with 97% (121/125) exercise sessions completed. Nine participants (56%) in the usual care group opted to participate in the delayed intervention. Among participants who completed the intervention (n=18), 88% reported enjoying the study, 100% would recommend a similar intervention to other young survivors, and parents rated participation as either not burdensome (56%) or slightly burdensome (44%).

Conclusions: Preliminary results support the feasibility and acceptability of telehealth delivered exercise programs in childhood cancer survivors. Following further investigation, digital health approaches should be considered to support young survivors with engaging in physical activity and exercise as part of survivorship care.
